    How big is the tank?

    Any power failures?

Any chemicals added to the tank?

2 things come to mind, high temp low O2 and high nh4/no2

How many fish and how old is the tank? What filtration are you using?

I wonder if the tank size could not be a contributing factor as well... 120L sounds a bit small for a tang keeping in mind that live rock would displace a bit of volume too.
