Please help ORI by completing this survey about your shellfish collection during the last three months. The survey provides us with basic information about the sports fisheries in KZN and is crucial for sustainable fishery management. Thank you very much to everyone who has completed any of the previous surveys!

This survey is on your collection efforts from 1 April 2015 to 30 June 2015 only. If you collected crayfish (East coast), mussels, oysters, mud crab, cracker, sea lice, octopus and redbait OR had a permit to collect these animals during April to June 2015, please complete a quick and easy online survey at ShellfishSurvey. A smart phone friendly version of the survey can be accessed at SurveyForCells. Your information will remain confidential and is only used to calculate fishery statistics. I have tried to make the survey more user-friendly. Please let me know which you prefer, this or the previous version of the survey.

All participants in this survey will be entered in a lucky draw that will be held in January 2016. The following prizes can be won:
· First prize: Two uShaka Platinum Passes allowing free entry into Sea World and Wet 'n Wild for a whole year
· Second prize: A Sea World Meet a Dolphin experience for two people
· Third prize: A dinner for two at the Cargo Hold restaurant (subject to availability)
· Two consolation prizes: Two double tickets to uShaka Marine World (SeaWorld and Wet 'n Wild)
· Another two consolation prizes: Two double tickets to Dangerous Creatures

If you received this survey in the mail during the last few weeks, only complete the mail questionnaire or the online survey, not both. You may only complete the questionnaire once per survey, but are allowed to participate in all of the 2015 surveys (in January, April, July and October 2015) and improve your chance of winning a prize. Please complete your personal details in full and provide a phone number where you can be contacted between 12-31 January 2016 during office hours. Thank you very much for your help.
