Just look at a hippo wrong and it gets white spot. Hopefully the cleaners will sort him out. Is he in a reef tank?

My regal is healthy and has never had white spot. The key is to fatten them up and obviously keeping stress factors down. But that should apply to any fish.
Keep up with the res-q-med at the correct dosage level and times. Hopefully he will be ok. Also remember to suspend any carbon running in your tank.

The garlic will not help. I used res q med before and did not get results. The cleaners do not eat ich from the fish and will do nothing.
Set up a QT and pull all the fish. Read up on Tank Transfer Method. The alternative is to use Cupramine.

Keep your main display fallow for 72 days. Only way to ensure you are rid of Ich.

    Lastly QT all future lifestock.

No. But HLLE has been linked to exessive carbon use or where carbon is not properly rinsed and the fines get in the water colomn.

    Carbon does however remove medications.

UV willl only kill what passes through it in the water colomn. It will not eradicate white spot form the system.

Yellow tangs are. But you should always suspend carbon when dosing medication. It even says so on the res-q-med

The garlic is to entice the fish to eat. Eating fish have a much higher survival rate.
As for the res-q-med. I seen alot of success with it. As with all medication.
It depends on the correct and timely diagnosis of the disease and obviously treating it with the correct dosages and correct times for the specific water volume of the tank.

What temperature changes could cause the whitespot?

Low temperature fluctuations. Ie a degree shift down could stress fish. What is your day and night time temperatures

We are talking about activated carbon or charcoal in this case not vodka dosing
