Maybe he tried to eat the urchin and got poked? But can a long spine urchin kill a fish? Does he have any marks on him?

*EDIT*

This.

"A longspine sea urchin can kill fish. It won't be on purpose, though. What happens is simple; urchins point their spines towards any potential threats, including fish who are being spazzy and darting around. If a fish is scared and darting around, it could possibly skewer itself on one of the urchin's spines. I've heard of this happening. They aren't going to be doing that on purpose, although I suppose they could accidentally corner smaller fish and cause them to panic if they were large. Only something like a large longspine could do this, though.
What kind of urchin do you have? How big? Did the fish have any marks on it? If it was killed by the urchin, it would have puncture marks somewhere."

From here.
