If the fish is rubbing constantly, it could be gill flukes or a parasite of sorts. Also look where he is rubbing, whether it is the gills or a specific part of the body. If the fish does it every now and then then it could just be itchy like you and I get

Looks like it coul be one of the brisstletoth tangs. Does it have very faint horizontal lines and small orange dots on its face?
Check if the fish is always scraping the same spot. Could be flukes. Is it eating?
How long have you had it?

Then its most likely a striped bristletoth (Ctenochaetus striatus). Aggresive when they get bigger.
Flukes are small worms (trematodes) that attach to the fishes skin and/or gills and feed off the fish. Heavy infestation will cause the fish to die, usually by damaging the gills causing the fish to suffocate.
If it is flukes you going to have to treat it. Do you have a hospital tank?

    A. nigrofuscus usually has a black dot at the base of the tail.

6 line wrasse wont do anything.
If you cant treat it in a hosptial tank you could try Microbe-Lift Herbana. BUT need to know what is causing the itching before you can treat it.

A big bucket with a fizzer and a floss filter can act as a Hospital tank. Try get a shot of the fish we need to see what is wrong..

I have used Herbtana but use it together with artemis.

    Thats perfect !

A fizzer is also know as an airstone. A Floss filter is those old freshwater filters that runs bubbles though and have charcoal and floss in. BTW medicine and charcoal should not be used together as the charcoal would reduce the effectiveness of any medication!
