How did the head look before you left?
Other heads all look fine from the pic. Is there any slime around the coral?
    Tank parameters?

Odd. Have you seen any fish/pests picking on it?

Was just wondering if it was a recent purchase.
I've found them to be particularly sensitive to transportation etc.
    Not the case here.

Has the "flesh" totally torn off...is that just the skeleton...? Or has it retracted into the skeleton...?

The first time that happened to me I freaked out and bawled my eyes out as I couldn't figure WHAT happened...:confused1:..after days of thinking...it dawned on me...my Hubby done a water-change...:(..he sucked the flesh right off.....he denied it, but sadly I got to prove it 2months later when he did one off his water changes...:(..that flesh got sucked up within 10 sec's.....Hubby had NOOOO idea he was so close...anyway...that's one explanation....the other one is the fact that I had the same problem a few days ago...I woke up and found my Torch flesh comming off on one of the heads....I've been battling 29+ degrees and my Torch has been sulking bigtime...I kepted on telling my hubby a few days before that the one head is suffering...it turned out to be that very head...so I don't know if that could be you'r problem...just thought I'll share...

None yet, hoping I will find out soon, but the frogspawn is looking good, it’s really plumped up a lot now, I will take another photo shortly.

I've had no success with the 2 pieces of this type of coral I've tried.
Hopefully you do better than me.

On both my pieces, the loss of 1 head was shortly followed by the loss of the others.
They seem, in my experience, to be particularly sensitive to bumps/knocks or unfavourable water conditions - highish nitrates, bacterial blooms or any form of parameter instability.
