Maybe a mantis shrimp is hiding in your rock somewhere.

What clean up crew do you have? Any big bristle star fish? Size of your tank?

If a fish dies your clean up crew would take care of the corpse very quickly, obviously depending on their numbers.

Also what was the size of the orange shoulder?

i have a few star fish two small cleaner shrimp and about 5 or 6 hermits but none are bigger then a R2 coin. there are a few biggish bristle stars could they be killing my fish or would they already have to be dead or dying for the bristle stars to kill them. the orange shoulder was only about 6cm. but the thing is i saw him yesterday and he was eating and nothing seemed wrong the tank is 1.8x600x600.
