Might be a bacterial bloom of some sorts

Well can be part of the cycle process as the tank age. How old is your tank. New tanks thats few weeks old might still have a bacterial bloom. What im saying is a tamk does a couple of cycles till it matures. Have you added somthing new recently?

Might be that. Are you dosing anything that boost your bacteria like vodka, vinegar or Special blend, if so cut back a little, also remeber any big change and your tank will do a mini cycle

I'm not dosing anything I did dose bacteria after I added the sand. I siphoned most of it out and did a 10% water change earlier on

What substrate is it you you using ? Looks synthetic? sounds like it was used in a previous system. Was it washed or put straight over.

How thick is it layered? If too deep it could be going anoxic which is very dangerous.

Be careful disturbing the substrate as it will release the toxic build up.

I lost a lot of fish to something simular...

If you use playsand with low flow then you sometimes get sulphur buildup, which is black/grey and gets darker the deeper the sand is. Not a problem in small areas, but if your whole substrate is like that then you want to increase flow.
